GETTING STARTED
Contact your practice to request to enroll in the patient portal. All you need to do is provide an email address.
Once you receive the enrollment invitation from Hometown staff – click the SIGN UP! button.
Enter your zip code and date of birth
Create your security details:
- Create a username
- Create a password
- Select a security question and type the answer to the security question
- Enter your phone number and select phone type
- Click the Enter Portal button!

We understand that your time is valuable. The Portal is an online tool that helps you manage your healthcare easily and securely. Time-consuming tasks become simple by clicking on the Patient Center tab on our website.
ACCESSING
The portal does not have a mobile application. Patients can use their mobile phones to access the Practice’s Portal website via an Internet browser such as Chrome or Safari.
FREQUENTLY ASKED QUESTIONS:
Is my information safe?
Yes. Portal passwords are encrypted, only you and authorized family members are able to access your information on the portal. The Patient Portal comes with a ‘time-out’ feature that protects your information if the portal page is left open.
Where does my health information come from?
Your information within the portal comes from your health center’s Electronic Health Record, ensuring you have access to the most accurate, up-to-date information possible.
Who can communicate via the NextGen Patient Portal?
You and your authorized family members can exchange messages with your care team between visits.
Are there set hours for using Patient Portal?
You can access your portal 24/7 from a computer, smartphone or tablet.
